Failing Areas in PoE2

So we all have seen that if you fail a boss or guardian or map it locks and you have to find said encounter again to complete them. But does anyone know for sure what is considered failed? Is it if you die once in said encounters is it then a fail? Or do you get all your six portals to try to complete said areas and once your out of portals you have failed then? (like in PoE1)

no one knows that fur sure , it was just not said .

but i think the whole idea off locking maps off when failing them is stupid.

one of two things will happen.

1. People revolt and map failing dosnt make it to 1.0

2. tryhard nerds embrace it like they do for XP penalties , and we are stuck with it forever .
you have 6 portals you can leave and reenter but this is just to limit the amount of items you can carry out of a map. If you die the map becomes inaccessible no matter how many portals you do have left. If you run out of portals before defeating all rare and unique enemies then that should counts as a failure as well. Not sure about that one but in order to succeed a map you do have to defeat all rares and unique enemies.

I think it is rather rough decision but we have to wait and see how difficult the content will be. How well the defensive layers will work etc. If it is tuned giga hard and majority of the playerbase lose like half the map they enter I can see them changing it.

what happens if a not so great player fails all the maps? they just don't get to develop the atlas talent trees and are stuck with tier 1 waystones? that doesn't sound like a fun end game.
