omen of amelioration did not work

was at 25 percent 99 and had omen in inventory , died , am at 15% and omen is still in inventory.....grrrrr

Did you have any other omens in your inventory? Either from previous maps or looted during the map. Once any omen is consumed in the map, another one cannot be activated in the same map. So if you had one that procced already for reaching 25% life, then Amelioration cannot proc in the same map.
