sepulchre blight map phantom damage bug

this is the third sepulchre map i've ran in the last hour where it'll be a long duration after blight has ended and will suddenly take very random damage, about 25% my health bar. it's instant, almost like my life bar just randomly clips off the 25%. there's no animation for an attack, no mob, nothing. just walk into a random spot to try and pick up items and life disappears.

im not sure if there's some sort of issue with maybe the mods i ran on it, so i'll leave a screen shot with them just in case

also to add onto this, the damage that's taken isn't a DOT and only affects me and not my minions at all considering we're all standing/walking into the same spots and i pay very close attention to my spectre/animate guardian life bars.
