I completed the map, opened all the strongboxes, opened the abyss and finished it, opening the trove, and did everything but the reward box will not spawn. The only thing left in the map are the packs of restless dead that keep respawning and messing with the monsters remaining count. I ran around the entire map more than 20 times making sure there wasn't a strongbox or a rare monster I missed somewhere. Here are the mods:
