Hello,
Recently we have experienced the bug with elemental reflect immunity failing to work during a map that had the refelcted damage modifier appiled. Despite having active Minor God Soul of Yugul 50% reduced reflected damage as well as the mastery passive from Elemental Mastery - 60% Reduced Reflected Elemental Damage taken, two members of the party got wiped out instantly with reflected damage. There were neither any monsters nearby, nor any skills like flame dash were used to cause that, and party attacked for the edge of the screen, while all were affected by a secondary condition which I suspect responsible for this - we had Acceleration Shrine active, with Atlas Skill Tree focused for shrines. It's worth mentioning that there were also Headhunter buffs affecting the attacking team at the time of event.

Was there anything that can increase damage taken? Shock (assuming no Purity of Elements), Covetous Shrine, Jugg stealing Zerker Aspect of Carnage, etc. Having a 15% shock with 110% reduced reflected will still net some reflected damage and require more than 110% reduced to offset.
There's tons of reports for dying to reflect damage every league, but to my knowledge they all found some other culprit to the deaths. If it was broken like you're suggesting, there would be a significant more number of reports. Small sample, but I ran like 30 8-mod maps with reflect elemental last few days with the same combination to reduce reflect, but I'm also ailment immune, no Covetous, and no HH or other inc damage sources and no issues yet.

Honestly, it's hard to say, we had ailment immunity, that's for sure, so shock shouldn't be an issue. And it's not the first time this happens with Acceleration Shrine, and I do suspect this to be the problem, cause projectiles are killing entities offscreen (even screens away) and this is instantly reflecting dmg to those that fired them. I'd say this might be related, cause in all the recent cases we had issues only when there were shrines active, including the Acceleration one.

Interesting, would explain why I'm not impacted as I'm not playing a proj skill. It's possible there is a hidden range on reflect immune and if they are invested enough into proj speed (via gear or HH buffs?), then they are killing way beyond that hidden range?
Depending on how invested your group is in isolating the bug, should be fairly easy to replicate. Stack even more proj speed for the test, run all shrine scarabs, and run through the map until you find the Acceleration shrine. Attack a bit, prove not dying, then click shrine and finish map fetching for a reflect death. The extra investment to proj speed might be enough to simulate the proj speed gained from HH if intermittent reflect deaths.
One other thing I didn't consider.. were the people dying using Haunted wands? I've not fully gone through the list of haunted mods, but curious if something in there could be a culprit with the shrine. Probably unrelated if 2 people are dying.. unless both players get the same modifier in the same zone.
