My friend cant play the game by server connection problem

somebody knows why my friend is having spikes from 30 to 150ms when he is not in the town? every "pve" map, where u can use abilities is having this issue, and its only in this game, we play a lot of games and 0 lag issues, even he start to having this problem since 3.24 and he quit hoping to 3.25 comes out without the problem but thats not the case.
We dont know what can be happening, if its any way to solve or at least know where its the problem... we tried turning of firewall, changing net config on poe menu, nothing happens
