Nameless Seer Not Appearing With Final Pack Spawns the Nameless Seer on Death Mod

Hello,

As the title says, I got this Arimor mod in Burial Chambers. Final monster happened to be in the boss room, but nameless seer is nowhere to be found. I ran through the entire map, was not able to find him. 0 monsters remaining on the map.

Same issue (0 monsters, backtracked three times to see if I missed him), in Maze of the Minotaur map.
Sacred grove spawned, didn't find him there either.

Same. In the campaign in act 5 Ruined Square. I have the “final pack spawns nameless seer” on my lantern. Killed everything, backtracked over the entire map 2 more times. No nameless seer.
I was stoked too, it was the first time I’ve gotten it, and now I get jipped out of a potentially good piece of gear.
