Game stuttering making the game unplayable

idk what the issue actually is, i have tried all of the methods i have found to fix the issue and it is still unplayable
the game will stutter very frequently with the game saying my cpu latency is around 400ms usually but it has jumped to over 4000, both numbers meaning my pc should have blue screened, most of the time the shader and drive bars will both max out
my cpu usage will never be above 40 percent when this happens
i tried the method of setting my cpu utilisation to 100 percent constantly, this did not affect it
i have reinnstalled
i have cleared caches
another issue is that the game will only display over 0.5 frames a second if it is in full screen, both windowed and windowed full screen will slow to this after a couple minuets of playing on them
