Flask disappeared

I just logged in and while doing an Ancestors trial I noticed my position 1 life flask had disappeared. I had a Forbidden Taste in that slot and haven't moved it or done anything with it or any other flasks. I've checked everywhere and it's completely gone. One thing that struck me as odd is that my position 2 flask was in it's place, almost as if it was overwritten by it, with the second slot being the empty one.
