Need help with dying breath build.

i have always wanted too make a build fully taking advantage of the unique dying breath.
Haven't scene any lists using them and i really need some advice.
haven't refined this in anyway and mostly a ruffshot of what i want too do.
as of now it's going in the way of hybrid srs,any suggestions or advice would be fantastic.
