Path of Exile Performance issues patch 3.23 Affliction

I can't understand why de game is so laggy, like I've used geforce now to play the last patch and it was FLAWLESS...

But after this new expansion i cant play propperly, there are a lot of spikes, shaders are constantly up to 100%... I've tried to switch from DirectX11 to DirectX12 or even Vulkan, the lags still persists, the maps take long to load and they dont even load propperly

You guys know how i can fix it? Or should i just message them?
